Over the last three years the Center of Medicare and Medicaid (CMS) rating system has included three triple weighted clinical pharmacy measures related to medication adherence.
- Hypertension medication: angiotensin-converting enzyme (ACE) inhibitors or angiotensin II receptor blockers (ARB)
- Diabetes medication: oral (excludes insulin)
- Cholesterol medication: statins
As providers you are evaluated on whether these patients fill their prescriptions 80% or more of the time.
Our pharmacists are trained to identify and concentrate on these three groups of chronically ill patients. Using actionable clinical tools to identify patient’s total body burden of medication a Pharma-MED pharmacist can acquire a more accurate view of how adherent each patient is resulting in a more effective consultation.
